You can pre-register your "Medical History" with Bedford Surgical Associates, Inc. before your first appointment via
 Turbo-Doc Electronic Medical Record. 

Contact our office at 814-623-1002 and provide us with an email address.  You will be sent an email, which will read :
YOUR NAME Please register with Doctor:  David C. Faber, MD. 

Open the email and click on the link "Online Registration"
which will automatically login to your personal registration.
The email will provide you with a username and password.  If automatic login fails, click Login and enter your username and password as specified in your email.
